Tony Mok and colleagues1 report improved overall survival with first-line pembrolizumab immunotherapy over chemotherapy in patients with advanced non-small-cell lung cancer with programmed death ligand 1 tumour proportion score of 1% or greater. The hazard ratio (HR) for the full cohort was 0·81 (95% CI 0·71–0·93; p=0·002) in favour of immunotherapy.1 HR analysis assumes proportional hazards, namely that the ratio of two hazard curves is constant over time. Given the intersecting curves at 8 months, this assumption was clearly not met, severely limiting the clinical interpretability of the reported HR.
